What is Social Prescribing and what do we do ?

Local agencies and professionals can refer people into the SPRING social prescribing service. People can also refer themselves into Spring. 

Once you are accepted onto the SPRING program, the Social Prescribing Link Workers will take it from there - they give people the time and support to focus on ‘what matters to them’ and take a holistic approach to their health and wellbeing.

They connect people to community groups and statutory services for practical and emotional support.

Social Prescribing is suitable for a wide range of people, provided that they meet these eligibility criteria:

• • • • • • Live in Northamptonshire
          • Are over 18 years of age
          • Are diagnosed with at least One long-term physical health condition
          • Are willing to commit to the program and engage in positive lifestyle changes

Our Social Prescribers are fully trained and will arrange to talk to you about how you are feeling, and what you think might help you to improve your health and well-being.

They will recommend, and help you to access, a range of social activities.

They will support you and give advice by establishing what matters to you. 

They will work with you to connect you with local services to meet your individual needs and help you to find new opportunities.

Topics for discussion and review include:-

              • Work, volunteering & other activities
              • Money
              • Housing
              • Family & friends
              • Mental well-being
