What is Social Prescribing and what do we do ?

The SPRING social prescribing service can be referred into by local agencies, professionals or people can self-refer.  Our Social Prescribing Link Workers will take it from there, giving people the time to focus on ‘what matters to them’ and taking a holistic approach to their health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Social Prescribing is suitable for a wide range of people, within the eligibility criteria:

• Live in Northamptonshire
• Be over 18 years of age
• Diagnosed with at least One long-term physical health condition
• Be willing to commit to the program and engage in positive lifestyle changes

Our Social Prescribers are fully trained and will arrange to talk to you about how you are feeling, and what you think might help you to improve your health and well-being. They will recommend, and help you to access, a range of social activities, supporting and giving advice by establishing what matters to you, they will work with you to connect you with local services to meet your individual needs and help you to find new opportunities.

Topics for discussion and review include:-

• Your lifestyle
• Looking after yourselves (everyday living tasks)
• Managing your symptoms
• Work, volunteering & other activities
• Money
• Housing
• Family & friends
• Mental well-being
