RESTART

RESTART is a team of doctors, nurses and physiotherapists specialising in patients with respiratory disease. It is based at Northampton General Hospital. The team's aim is to provide individual, quality home care to patients with acute exacerbation of COPD.

ASSESSMENT IN HOSPITAL

On your arrival at hospital, a full medical and nursing assessment will be performed. A respiratory specialist nurse or physiotherapist, who is specially trained to care for patients with respiratory disease, will see you.

Some patients may need to stay in hospital, but for those who can be cared for at home, an individual plan of care and treatment will be provided. Your GP will be informed of your discharge from hospital.

CARE AT HOME

Patients who can be treated at home will be discharged under the care of the Restart Team. The nurse who assessed you in hospital and other members of the team will visit you at home daily for the first 3-5 days, followed by telephone calls and further visits until you have recovered from the exacerbation of COPD. District nurses may also be asked to visit you at weekends. Whilst at home you will remain under the care of one of the Consultant Chest Physicians at Northampton General. It is important to remember that during this time the hospital is responsible for your care relating to you respiratory disease and not your GP.

Your care at home will consist of help with medication, monitoring your condition including blood pressure, oxygen saturation, and respiratory rate and assessing your symptoms. The respiratory team will provide advice and support for you and your family. You will be discharged from the service when your condition is stable and returned to the care of your GP (maximum 10 days under Restart care). The RESTART team are based at Northampton General Hospital so cover the South of the County.

ROCKET

Based at Kettering General Hospital and covering the North of the County,The ROCKET Team are a respiratory outreach team that similarly supports patients with conditions like COPD.
