Patients leaving hospital after a long stay often feel anxious or uncertain about going home to an empty house, where for long periods of time they may be alone, afraid of falling, or family Carers feels they may struggle to manage on their own. Our service provides the support and confidence to empower them to return home.

Patients may be going into hospital for a procedure where they need observation afterwards. If a patient lives alone then they would be admitted overnight in these cases.
