Have you had a heart attack or living with heart failure?

This group is being delivered as part of the ICAN (Integrated Care Across Northamptonshire) Project which aims to help people long term.

You will have access to:

• Expert advice and guidance from an NHS consultant
• Education and information from guest speakers
• Wellbeing Activities and exercise
• Peer Support and friendship
