The aim of Age Well work is to help older and frail people to choose well, stay well and live well in the community. By improving how and when patients can access the care that they need. Age Well teams where possible, see people in the convenience and comfort of their own homes or via online video calls facilitated by Age Well workers in your home.

Age well teams are linked with your GP surgery and made up of health, social care, and voluntary sector workers from a variety of local organisations. All members of the teams are trained to support you and can make sure you gain specialist advice from the right person.

Northamptonshire Carers has Age Well Coordinators based across Northamptonshire. 

Age Well aims to bring about the ideological and working practice change necessary to properly join health, social care, and wellbeing services.

The multi-disciplinary approach emphasises to both patients and professionals that health is more than treating health concerns and looks at social issues, including housing, social isolation, equipment, and access.

Eligibility

  • Be Aged 65 and over.
  • Have one or more health conditions.

What can Age Well support me with?

  • Develop and agree a support plan to meet your needs. 
  • Ensure you have any mobility and monitoring equipment you need at home and if not the are able to prescribe low level equipment, negating a waiting list and reducing risk of falls etc.
  • Arrange a review with a GP, pharmacy colleague, and a nurse to discuss the issues that matter to you around your health.
  • Support you in accessing local social and or exercise groups to get specialist advice and guidance around specific health conditions.
  • Link you with someone who can review the benefits you are receiving to make sure you are getting the ones that are available to you. 
  • Ensure that those caring for you are supported and accessing support service available to them.
  • Blue Badge applications.
  • Link you with a befriender service if you choose.

What does this support look like?

If you agree a member of the Age Well team will come to visit you at home to talk in more detail about the most important things to you and to understand more about your needs or the person’s needs that you care for.


As part of the support offered your Age Well worker may arrange a virtual or in-person call with the GP lead for the Age Well team and other specialist team members. Your Age Well worker will be with you to support you when the call or appointment takes place to ensure the patient and their carers at the centre of the consultation.
