The successful candidate will be able to offer a personalised and supportive to individuals whilst being able to work to targets.

Hours: 37hrs per week Based at: 123 Midland Road, Wellingborough, NN8 1LU   Salary:£22,183 - £24,491 PA Contributory pension and health benefits package

Social prescribing empowers people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me,’ take a holistic approach to an individual’s health and wellbeing and connect people to diverse community groups and statutory services for practical and emotional support.

Social Prescribing Link Workers have the very important job of taking service users by the hand through the journey of improving their wellbeing; helping to support clients as they learn how to navigate the community on their own.

Link workers also support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local diverse partners.
