Northamptonshire Carers is a voluntary organisation, registered charity and network partner of the Carers Trust, supporting informal carers countywide.  We are regulated by the Care Quality Commission to deliver domiciliary care. We are looking to recruit  salaried Carer Support Workers for our daytime hospital discharge team.

Daytime Hospital Discharge Care Service

This post is a social care development role within Northamptonshire Carers to support people with are needs and their family carers to remain as independent as possible at home, following hospital discharge.

To provide support across all regulated care services, provided by Northamptonshire Carers.  

Hours are 37.5 hours a week over a shift pattern of Monday to Friday & alternate weekends either working 07.00 to 15.00 or 14.00 to 22.00. Including a 30-minute unpaid lunch break.

Bank holidays are inclusive.

Flexibility to provide cover for colleagues is essential and dependant on the needs of the service.

To provide support and assistance to carers and people with care needs. This will involve the provision of personal, emotional, and social care, always respecting confidentiality within an equal opportunity’s framework.

Liaison with other professionals including health and social care providers as part of a multi-disciplinary team

Salary: hourly rate £10.63 PH (£20,728 PA)

Mileage allowance, contributory pension and health benefits package

Full training will be provided. There may also be an opportunity for an apprentice development pathway leading to a formal qualification.

The posts require individuals who have an understanding of the needs of those with care needs and their informal carers. A flexible and person-centred  attitude is essential as is an ability to use your own initiative and communicate effectively. You will need access to a vehicle and a driving licence.
